Qualified wholesale customers must have a copy of this Wholesale Application form on file with us, as well as a copy of a valid, State issued business license pertaining to the jewelry industry. Please fill out the form on right and submit it to us to receive wholesale pricing information.

By filling out this form you certify that all the information is accurate and valid and that all items purchased from RG Clay Creations from this day forward will be purchased exclusively for resale.
